Simple Ajax Email Contact Form

This is a simple contact form that submits the users name, email address, phone number, web page and comment to any email address you specify in the module settings. The form is submitted via Ajax so the page never has to refresh. After the form is submitted a simple thank you message is shown to the the user and the form is reset.
